You can choose between registered and unregistered providers for support coordination services.
To locate registered providers, use the Provider Finder tool:
- Search under the registration group ‘Assistance in coordinating or managing life stages/transitions and supports’ for providers offering Level 1 Support Connection or Level 2 Coordination of Supports.
- Search under the registration group ‘Support Coordination’ for providers offering Level 3 Specialist Support Coordination.
You can connect with a support coordinator by submitting a request for service. Your planner can assist you in preparing this. Requests can be made:
- Through the myplace portal for registered providers
- By email for unregistered providers
- Directly to your chosen support coordinator
Working with your support coordinator
After connecting with a support coordinator, you should meet to discuss the services they will provide.
Support coordination can be delivered in person, over the phone, or via video, depending on your preferences.
You and your support coordinator should agree on a service agreement, which includes:
- The services they will provide
- Your and your support coordinator’s roles and responsibilities
- Processes for resolving issues
- Procedures for changing or cancelling services
You decide who can access your plan. Discuss with your support coordinator what information they need to best support you and how you prefer to communicate.
Changing your support coordinator
You can change your support coordinator at any time.
If you decide to make a change, discuss this with your current support coordinator first. Follow the process outlined in your service agreement.
Your current support coordinator should assist in finding a new provider. Before transitioning, you may need to update or end your service booking with your current coordinator. They can help you with this process.
Once the transition is complete, you can create a new service booking using your remaining support coordination funding.
Your current support coordinator must provide a report detailing key information for your new support coordinator. This report should also be shared with your new provider and the NDIA to ensure uninterrupted support services.
